Submit and Confirm

Once you’ve filled out the packet, email the completed forms to extraordinarykp1@yahoo.com. We’ll review your submission and follow up to confirm your child’s start date.

What You’ll Need

A record of your child’s immunizations is required before their first day of care. You’ll also have 30 days from the start date to provide a current health examination report signed by a medical provider.
